Chuck Lake Distinguished Service Award

The Chuck Lake Distinguished Service Award is given to an individual who has demonstrated a commitment to high standards, who gives his or her best efforts to every task, and who is respected for his/her competency. The Award is named in honour of Chuck Lake whose fine personal qualities -- tact, acceptance, energy, creativity, sense of humour, and positive approach to life -- have so enriched his colleagues. This award is given to an individual whose contribution has been to strengthen the Nova Scotia Council for the Family.

Criteria and Nomination Process

Letters of nomination should include the significance of the nominee's contribution to the Council and its work.

The nominee must be a current member of a Committee of the Nova Scotia Council for the Family and nominations can come from committees of the Council or Council Board representatives.

Performance

Write a paragraph or two on the performance of the candidate using the following headings as guidelines. (Note: It is important to be as objective and specific as possible and to illustrate by example where possible the areas in which the candidate demonstrates outstanding performance.) Final submission to be a maximum of four pages, excluding support letter.

  • Creativity and Strategic Thinking (candidate tries new things, creative but not reckless, secure in the knowledge of the work of the Committee, advocates on behalf of the Nova Scotia Council for the Family)
  • Team Player - Leadership Qualities (leader among peers, respected by colleagues, and significantly contributes to the work of the Council)
  • Imagination, Initiative, and Perseverance in Accepting Responsibilities (time associated with the Council, responsibilities while there and what has been accomplished, e.g., a proactive approach in supporting the goals and advocating on behalf of the Council)
